If the bill is less than $25-30, then I give $6.
$30-50, then I give $8-10
$50+, then I give 30% whatever it is.

Where I live in California, the tax is 8 1/2 %, so I normal double that, then round up. If the bill is around $20, then I try to tip $4-5.

I don't tip by percentages. I tip by service. If the service is excellent, I didn't have to wait on my drink to be refilled, etc., I wouldn't think twice of tipping ten to twenty dollars.
If the waitress is rude, I have to ask three times for something, or she ignores me to the point where I actually get up and do it myself, then she's lucky not to get a phone call to the manager.
If I go to Sonic or something, I usually tip a dollar or two, because I've worked there, and I know how it is.
Again, depends on the service, and I'm from Mississippi.

The starting tip for me is 20%, and I deduct from there based on the quality of service. 15% really should be your minimum tip. There are very rare cases where servers deserve to be stiffed (0%) though, and I do that when necessary. If it is a place I frequent, then 25%-30% is standard.

Totally relavent to the quality of the service I get. Example: I once stiffed a waiter for not asking if we wanted anything else. My friend was working there (cooking and came out to see us) and he asked the waiter why. He said something to the effect of "I'm doing them a favor. Do you see how fat they already are?" Then we went across town to get our desert and I tipped like $5 on a $3 tab because the waitress was acting like we were the only customers who mattered (place #2 was actually busier at the time) this was like over a decade ago, so that's why the cheap deserts, but I hope you get the picture.

I'm in TN. Normal here is 15-20% depending on restaraunt and service.
I waited tables here during college (6 yrs ago). I averaged closer to the 20% range. I was in a very casual steakhouse and had a pretty regular customer base.

bad service - maybe 2%
okay service - 10%
good service - 15%
really good service - 20 - 25%
once service was so good I tipped like 35%
if my son makes a mess for levels okay and above, I will throw in a couple of dollars. I'm glad he's outgrowing this stage
I worked in restaurants for years, I have very high expectations, rarely are they met

Normal is 10% - 15%
If the service was above and beyond, they get about 20%.
The night of my rehearsal dinner, we tipped exactly half the cost of the bill. The service was unlike anything we have ever received. We also sent thank you a thank you note to the manager letting him know how wonderful his entire staff was to make sure that our entire party was properly and satisfied, from the adults to the children.
