Yes they are all pressurized. Every pressurized aircraft I have ever worked on has had a pressurized cargo hold.

Boeing 727, 737, 757
Convair 340, 440, 580, 5800
SAAB 340
C-130 Hercules (entire fuselage, with all doors closed).

I was on the design team of the HS 146 for the cargo bay and that hold was presurrised. The cabin floor was supported on A frames from the hold. The whole structure was a series of hoops covered in the skin. I don't know if this is true for all aircraft but I would expect it to be.

I would think that to make allowances for metal fatigue it would be easier to pressurise the whole fuselage rather than just the top half.
Most airliners also have a separate heating system that is only brought into use when live animals are being carried as cargo.

Cargo aircraft deriving from passengers jetliner have pressurized cargo bay, because the fuselage is designed to be pressurized while the wall between cockpit and cargo zone is not designed for this.
So it's simpler to keep pressurized the entire fuselage instead of change the cockpit cabin design.
For military cargo aircrafts I don't know exactly, but I think that their bays can be pressurized or not, in fact when they carry soldiers they must pressurize, but paras must jump from high(HALO) the bay can't be pressurized while the door is open.

Most airline cargo holds are pressurized and some are heated and some are not as much. Some Aircraft and just one hold that they can put live animals in. I do know it gets cold in there, my suit case has come out of there very very cold.
