No sleep for 4 days would have a devastating effect on the body. anyone this happens to would need help. The body will stop functioning as it should. The systems would shut down and if there is a drug problem its bound to all come to end sooner than later.

Tension first of all.
Also, almost schizophrenic type delusions and hallucinations (visual and auditory).
Physically, your body will be more tense and your nerves more on edge.
Psychiologically, much slower and so you shouldn't be doing things like driving.
Different in different people.

There's a movie called Insomnia - Robin Williams - about a detective who can't sleep which clearly displays what happens when you don't get enough sleep.

I'd say you'd become extremely tired, start drifting in and out of consciousness and things would seem dreamlike and unreal
