Every 3 months, or when you see the bristles beginning to splay.
I put my toothbrush in the dishwashwer to clean it well. Make sure not to use the "heat dry" function. It also helps to soften up the bristles when you buy a brand new toothbrush.

I get a new one every time I buy a new tube of toothpaste but I do pour alittle rubbing alcohol on it several times inbetween to disinfect it.

every 3 months, or when its grotty or damaged. washing it in savacol or listerine works good to disinfect it. but generally, 3 months.
