Perhaps that you're younger than she is, and she's trying to be 'nice' - or she's being patronizing. Without context, hard to tell.

You could take it two ways. One way is that "little friend" to her is meant as a term of affection. However, it could also be taken as condescending - in other words you are a "little" friend as opposed to a regular friend. Meaning you are less than.
Only you know the situation - are you friends with this lady - or just casual acquaintances?

You must be in the deep south or she is from the deep south. It is an expression older ladies in the deep south use when talking about or to people a lot younger than they are who they are fond of.

Maybe she was trying to convey that you weren't her "friend" in the sexual department.
I've been called that bt women I've been intimate with, as in "oh, he's my friend" instead of calling my what I really was, a screw-buddy.
By labelling you "little friend", I'd say it takes away any notion of sexual intimacy.

Take it as a compliment. It was a term of endearment. Like calling you "honey."
