Any woman can scream Yes Yes but that doesn't make it an orgasm. Like someone else said if you have to ask then you didn't.

Guys take this soo seriously...consequently girls will often fake it because otherwise it will always be about, "Did you ***? Did you ***?" and if you didn't, he thinks it's his fault and it can really mess up the relationship. So women lie.

How can you tell? Despite what everyone here has said, you really can't tell unless she has been open from the start and already told you that she doesn't, or can't, or hasn't. Then when she finally says she did, you can probably take it as a fact!

You have already described it adequately. I wish to add that hurt is only in the beginning when hymn ruptures, if she is a virgin. Later sense of pleasure overwhelms all hurt. What follows is shrieks and cries in joy and ecstasy. The signs of orgasm are usually unmistakable.

The physiological signs are: 1) Is her skin flushed around her neck/chest/face? and 2) Is her clitorus very sensitive to touch after she has an orgasm? If she suddenly doesn't like being touched afterwards because it's too sensitive.. then yes.. she did.
