Here in the US, medical insurance costs are determined by a large number of factors. Geography and mortality tables are primarily used to determine premium costs. I recently got my Insurance Provider's License, and that was actually one of the topics covered.

In the US, average cost for an individual is $250 a month, average cost for a family (three and up) is $600 a month. Employers usually pay 80% of it. 3/4 of people in America HAVE health insurance, the vast majority of the rest are here illegally, own their own small businesses, or opt out of getting employer health insurance.
