Various sources credit Lieutenant-Colonel William J. Clear, or Lieutenant-Colonel William Casey, but the phrase is most often attributed to journalist Ernie Pyle

Ernie Pyle, a newspaper writer. It was a hook for one of his stories. Then as now the religious like to believe that God is watching over them.

Pat Tillman was an atheist in the foxhole

He gave up an NFL career to join the U.S. military and then was killed. Read about him at the following link.

I think in WWII. Some military people have now complained about this since they claim that they have been in this scenario and still not believe in a god.

As to the origin.
The term fox holes didn't come up that I know of until WW1
I'd be guessing that.
