Essentially western manners are an extension of day to day etiquette and manners (which are rarely adhered to these days anyway).

Leaning your elbows on the table, or "spreading your wings" (holding your elbows away from your body) is still frowned upon in more elite restaurants.

When having a multi-course meal, always pick up the cutlery from the outside and work your way through the courses to the cutlery on the inside. The waiter should remove the pieces you don't need to use.

Always lay a napkin on your lap (NEVER tuck it into your collar).

Don't start your meal until everyone has been served.

Worst case scenario, pause and look at what everyone else is doing. And if they're not remembering their manners, then use what you can remember, and you will outshine them. :)

People here in the west that came from back east have the same manners you do.
those of us who were raised here but their parents are from back east have slightly less manners and those who are 3rd or 4th generation westerners have little to none.
They eat with their mouths open, slurp their soup and generally make disgusting noises while picking the lettuce up out of the salad bowl with their bare hands instead of using salad forks.
they lean on the table with their elbows and come to the table without wearing shirts.
In general, they are (i can't say it, i dont want to get a violation)

Look for a book by Emily Post--it'll have all the info you'll every need.
