I'm not sure about the #1 part, but the one place that immediately comes to mind is Harrison's Cave in Barbados. I only went once myself when I was very small, but I remember riding through them in a tram, which suggests that they are fairly extensive.

Their official website is here - http://harrisonscave.com/ - and says that they're currently closed till 2007.

The north coast of Barbados is also quite spectacular, there's a place called North Point (original, huh?) that's definitely worth a visit. Just look out for the sinkholes! And go at low tide if you want to go exploring the caves down below. Otherwise you can content yourself with hanging off the cliffs. Just don't fall off.

There are other rock formation/cave attractions on some of the other islands, for example Jamaica's Cockpit Country - lots of limestone rock. Though the draw here is probably more historical than anything else - it's the site of famous slave revolts and what not.

And there's the Gasparee Caves on the island of Gaspar Grande off the NW coast of Trinidad.

Can't think of any others though undoubtedly they do exist.
