yes my husband just did.. but he has to put my name on the paper as well. i mean he signed it but in the form it asks for mothers name as well.. thats for uk.but in babies pasport your or your partners name not gonna be written anyway

In Canada, yes ... I'm not sure why it would be different in the US, but according to the other post it is. Being on the birth certificate only declares that person the legal parent of the child. It doesn't negate them to notify the mother .. nor how they would even know where to begin to try is beyond me.

Who needs to sign it?
The parent or someone with parental responsibility should sign the form.
If your child is 12 or over they should sign and date section 6 (leave section 6 blank for children under 12).

No I think if both parents are alive both fill in parts of the form especially the mother.

yes he can he as parental right just like the mother.But you can go and see solicator to get a order from the court to stop hiim taking the child out of the country.
