Just a verbal thanks will do, if you see the boss. And the other anwerer is right, legally, if you recieve a bad reference, and it is not true, and you can prove it, you can sue the person/company that wrote it. Most companies are careful with what they say on paper. Of course, there are ways to get around that, a phone call does it.

I think you should send a nice thank you card with a handwritten note inside.

No gift is necessary unless he went above and beyond the call of duty to help you. Did you get the job? If so, maybe a small gift (like chocolates, cookies, etc) at Christmastime.

Handwritten thank you card.

Imagine you're at a meeting with your new employer and your ex-boss says "Hey Ambi - thanks for the wine and chocolates." A big thank you for a reference letter could sound like bribery.

The ex-boss' thank you is good karma for doing a thorough job on your reference.

I don't think you really owe them anything. You obviously did a good job and earned a very good reference. If you feel you should do something, a nice thank you card or email should be fine.

Send a nice thank you note or a card. I don't think you need to go over board on any gifts or anything.

You can give a quick phone call to say thank you. or just send a note. I would call. no strings attached. You could even just ask for voicemail.Even better.
