I some in some ways feel that some high end fashion designers only make clothing to fit smaller sizes...and if they do make a size large or extra large it can still only fit on a person who wears a medium size..

I feel this sends the wrong message to woman and young girls...

First they have to have tons of money to buy the clothing then they have to be a size 0 .....I know it they have the right to make clothing the way they want....i fell that it still sends a strong negative message

Interesting question. I think that you are correct that they should hold some responsibility for it. But there are other people involved as well. Like the people who raised these insecure women and men with eating disorders, who didn't teach them to be secure in themselves and that these models are unhealthy and are at an unattainable weight without doing drugs and/or being anorexic /bulimic. There is too much emphasis in the media, though, to be perfect. I see it when they take a beautiful woman with a beautiful body and yet they feel the need to airbrush her to make her look even more perfect. And how they require certain types of celebrities to be under weight to keep their career going. It makes these girls and guys think that this is how we are supposed to look like when it's not only just a fantasy but these models are practically cartoons, drawings, 100% fake from start to finish.
