Depends on your age, location and (as always) the money.
You could try:
"I've got two tickets to the (game, concert, etc.) and was wondering if you'd like to come."
"I heard about this place (what ever). You ever heard of it? Sounds interesting. Wanna go sometime?"
"Want to go out for a (coffee, drink, etc.)?"
"You know, I've been waiting on you to get around to asking me out. At this rate...I'll have gray hair and dentures. So, I thought I'd better take matters into my own hands. When are you picking me up and where are we going?"
Simple works best.
Be casual. Don't come on too strong or you'll look like a stalker.

Asmita S.:
In this day and age in which we live, do you really think you need to be innovative and creative about asking a guy out? If you see him every day, you might try this.Engage him in conversation, as usual, ask him questions about himself. Guys love to talk about their lives and themselves. Then you could casually suggest you get together so you can spend a little more time getting to know him. Maybe lunch somewhere. If you're feeling especially bold, then suggest dinner. If he is like most guys, he should jump at the chance. It's an ego thing with the male. I hope it works for you.
