It means they can't help it, it is part of a disease, it is who they are, and they can't stop. Like a pathological lire can't stop lying.

A crime is pathological if the perp is suffering from a mental disease or the crimeis so extreme that it would be considered VERY abnormal. Not to say that crime is normal...it just means that there was something so out of place that one might tend to think the perp had a mental illness.
