Like, distinguishing those girls who are all pretending to be scared to draw attention...

How do they prove that?

I would imagine for a real scientific test, they would monitor the heart and lung functions, as well as the brain. The physical reactions would determine if they were really afraid or faking it.

Well a true, blown out phobia will interfere with a person's daily life to some extent. i.e. continually avoiding certain places (elevators) animals (or places/situations where they might come into close contact with them ). You get the picture. I suppose you could do a test where the researchers recorded heart rate, sweating, and other physiological reactions to an object or place. You could also record data from someones daily experiences. As for those girls, well you can usually tell if one of those women (or men for that matter) are faking, as faking fear is not the only thing they do in order to attract attention.

You would go to a psychologist or psychiatrist for a diagnosis.

there isn't a medical test for a phobia, it's a psychological condition.
