Mou hitotsu = one more
Hoshii = want
and desu ka makes everything preceeding it into a question, so
Mou hitotsu hoshi desu ka? = Do you want another one?
Want one more?
Would you like another one?
etc...
In Japanese it's, もう一つ欲しいですか?
Japanese often omit the words for "You" when it's obvious already who is being spoken to. Kinda the same as our," Want another one?" in English.

I lived in Japan when i was a kid- all I can tell you is that the desuka makes it a question- this person is trying to ask you a question.
