answers all those questions and has links to all govt agencies.
some places still have them at the hospitals or churches but most in the USA do it through either the state or county..
it helps to know exactly where you were born.

i had to send in a copy of my license and proof of address and ss card and a signed request with place of birth and a small fee last time i got mine.
See if you can get two certified copies and put one safely away in an emergency pack. i wish i did.

locate the town or village you were born at and then contact the village offices and they will be able to connect you with the person or person you need to speak to in order to obtain a duplicate birth certificate.most likely you will have to pay a small fee for the copy

YOU CAN CONTACT THAT STATE REGISTRA..
LOOK UNDER STATE CAPITOL DEPT OF INFO AND SEARCH UNDER BIRTH CERT ECT.

OR CALL CLERK OF COURT IN COUNTY OF BIRTH WHICH IS THE EASIEST WAY.
