they are more close than people realise but usually meditation is self induced ,and hypnosis ,usually external,
but the meditation affect is very close to hypnosis especially when proctised under a guru with monotonous repetitive sounds
people uasually self hypnotise with out realising it as a survival /coping mechanism

Hypnosis is different from meditation but meditation can lead someone to self hypnosis if done improperly.
In a group meditation where there is someone leading you to something by conditioning your mind, may also result to some hypnotic state.
The real meditation however, if done without being lead to a certain conditioning can be achieve by merely silencing the mind and not picturing any image and trying your best not give so much attention to what will be passing through your consciouness or even subconsciousness until the point of feeling blissful within while you no longer feel anything within your body such as tireness, fatigue, irritations, anxities or impatience.

No. Hypnosis requires someone else to hypnotize you. Meditation is something you do alone.
Meditation is equal to prayer. They accomplish the same thing - just different routes. One is through a mythical god, the other is via subconciousness being tapped.

Hypnosis is an altered state of consciousness, according to Wikipedia.
Meditation is an exercise which involves consciously focusing the attention on an object for a variety of purposes depending what kind of meditation it is. It requires intention & improves with regular practice. The mental & physical benefits of meditation are extensive. This is a very large subject.
I meditate regularly.
