While a cake is baking, there are gasses that form tiny bubbles in the batter. If it's cooked through, these bubbles are frozen, which gives the cake the spongy texture.
If the cake is cooled before thoroughly baked, these bubbles collapse, causing the cake to go flat. There you have a "fallen" cake.

Cakes will fall if the oven door is slammed or something falls hard enough by the oven and if it is pulled out to often to check if it is done. and if you take it out before it is done. using a toothpick to poke it while baking sometimes makes it fall. the center /middle falls in .
