Rt. 495 is the Long Island Expressway - it runs East and West.
To get to the GWB from 495 take the Fairview Expressway (Rt 295) North, (this used to be exit 27 when I lived there), go over the Throgs Neck Bridge and into The Bronx. Follow signs to the Cross Bronx Expressway (Rt 95) WEST, which will take you right over the GWB.
One word of advice friend, avoid rush hour if at all possible!!!

Lng Island Expressway to Exit 27N Clearview Expressway towards to Throgs Neck Bridge(toll is 4.50) after you cross the bridge you will see signs that say I-278 Triboro Brigde(right lane). Take the Bruckner Expressway and do not exit. After you cross the draw bridge the highway makes a cruve south.Then about 5 mins take the right lane take says Major Deegan Expressway I-87 North. Then take exit 7S Cross Bronx Expressway George Washington Bridge I-95.

As one of my rules of life is to avoid the Cross Bronx Expwy at all costs, I suggest this alternative:
Exit 22A to Grand Central Parkway, Triborough Bridge to Manhattan to Harlem River Drive North to GWB
