Gee! You must really like her a lot! Those are all symptoms of puppy love. Listen! You need to get some self-confidence, first. Then the rest will follow.
Why not get some advice from some older buddies who have experience in dating? They can probably help you with a few good pointers. And remember, she is probably just as nervous as you are. So, just relax and have fun with her. Next time you have a date, before you see her, think up some good topics to talk about with her.It will help you feel less anxious. LOL

Thats called "Situational Anxiety". The best way to deal with it physically, is to push back your shoulders and keep your back and head straight. That'll release the muscles in your neck and normalize the blood flow to your brain.
As far as the mental side (studdering, can't think what to say, etc) try thinking of something that makes you angry. The Anger will help block out the fear.
Also, remember to breathe and concentrate on breathing normally.
Asking girls out is like everything else that's scary: The more times you do it the less scary it gets.

The first thing you need to do is relax. Remember, she is much like you, just another human being, not some supernatural goddess. Everyone has self doubts sometimes, but you have to keep in mind that there is no valid reason to think of someone else as so great that they should ever make you nervous. Once you truly believe this, your nerves should dwindle and you can just be yourself.

I don't know you that well to give a detailed response but you should try visualisation. Imagine that you ask her out and she says yes - the whole aim is to not make you sweat buckets but to get you to be confident about it.
An auntie of mine used to say that "everyone sh*ts" and believe it or not even she does...in other words she is only a human being.

its easier said than done, but, don't be shy. don't think about it so much, you're stressing yourself out. talk about something you are comfortable talking about. make sure you give your input. its like talking in front of a lot of people. you are nervous at first but then when you get talking it gets easier.
another important thing is not to put on a show and be somebody you're not, unless you want to act like that forever. you don't want her to start liking the person you're acting like if thats not you.

You just have to work up some courage and be used to being around her. She is obviously very important to you, but don't get too attached because things, as you might know, don,t always go your way. Hope this helps!
