The most common mental illness which produces hallucinations and the inability to determine what is real and what is not is schizophrenia. It responds well to medications, but many people feel slow ans stupid when medicated and stop taking it. For a while, they're okay, then the delusions return. It's not a complete disconnect with reality, entering an alternate reality. It's more like attempting to incorporate the delusions into the real world. I know two people online who appear to have this illness, and it's obvious when they're stopped taking their medications. Each of them begins to believe that a man they have never met (a celebrity) is something he is not. One thinks he's her boyfriend, the other that he's Satan. These people are not trolls, and they have been in this cycle, which slowly evolves, for ten year or so, Fascinating, and sad.

Back in the 70s Patty Duke was the popular young actress with the Patty Duke show. She played identical twins on it. This site tells about natural cures for mental issues and has link to Patty Duke's book on her having bipolar.
They made a movie about delusions that math genius, John Nash had called A Beautiful Mind.
