The spouse's insurance carrier will not want to be listed as primary, you need to ask both medicare and the spouse's carrier and get a letter stating whom they recognize as primary..

If the spouse is medicare eligible because of age and the employer insurance covers more than 20 employees then the employer insurance is primary..if less than 20 employees then Medicare is primary.
If the spouse is medicare eligible because of something other than age(disability...etc) then 100 employees or more covered employer insurance is primary less than 100 Medicare is primary.
The above is only if the spouse is eligible for medicare.
If only you are eligible for medicare (no matter what type of eligibility) then Medicare is primary over your spouse's employer health insurance.
