Procedure might be the same for Delhi and Mumbai , u van get a passport application form some stationery shops and fromthe passport office , after that just fill the form and submit to any passport office ,rest is taken care of by them . This happen with me , i took it from Delhi.

Now a days you can obtain passport application from e-seva offices too instead of going to the passport office. You can fill it up and submit there along with your residence proof & 3 passport size photographs along with the amount charged for getting a passport. The amount is about Rs. 300
