If she has a bladder infection it could cause blood in the urine. Antibiotics would take care of that. If her behavior has not changed and she is still alert, purring when you pet her, and eating, drinking, and using her litter box normally, she is probably not in too much pain. However, she could be made more comfortable. She may need extra fluids, but a vet could assess that. She is old, but not necessarily at the end of her life. I've known cats of 24. So you needn't let her go just yet.

Bernadett why waste time waiting for these answers get the poor cat to the vet and let the professionals examine your cat. But you and your cat has done really well lasting to this ripe old age,you must have taken good care of him/her, so don't let it down now when it might be in pain or discomfort, take him to the vet now .Good luck lass, I'm sure all the cat lovers are thinking of you.

I must agree with others that say a trip to the vet's and if necessary doing the almost unthinkable would be the kindest last favour you could do your honey.
I know the thought of the loss hurts horribly, if it must happen, but not only will you be saving her immense pain, you will also be saving yourself the agony of watching her die, and the feelings of guilt.
If it is an infection or someting else, she can be back on the path to health and maybe avoid a fatal complication.
Huge Hugs, Consolations, and best thoughts in this difficult time,
Chuck

why haven't you taken her to a vet? Cats do not display pain by groaning but by purring so if she is doing that she is in pain. It might be a simply kidney infection which could be cleared up with antibiotics.
If this was your elderly granny with blood in the urine, would you be posting here about it or phoning the doctor???

Based on her age and weight loss, it sounds like it is her time to go.
If she has a kidney problem, such as stones, or a urinary tract infection, then yes, she is in pain ... and it would be kinder to have her put down quickly, rather than have her pass away slowly in pain.
Most vets these days will first give her a tranq, which will take away her pain and give you a few minutes to hold her peacefully and say your goodbyes. Then, when you are ready, the vet will administer the lethal dose that will allow her to simply fall asleep peacefully.
20 years is a long time to have a loving companion; I know you will feel her loss greatly ... my heart goes out to you.

It sounds like it might be time to have her put down- otherwise her suffering will get worse. Twenty is a very old age for a cat, so she probably won't get better. Why don't you take your cat to the vet and ask his or her opinion about it. Sorry to hear about that, it's sad to be losing a kitty.

Our cat had this problem and had stones in his bladder or somewhere which was causing the blood. Your cat is pretty old though and it may just be her time. Your best bet is to take her to the vet. It may be more humane to put her down before she gets worse.

Congrats on getting your cat to that age. My cat lived to be 18, she died in peace, but she didn't have any blood in her unrine. She could be in pain, especially if it's something to do with her kidney's, bladder, or liver. I think that the best thing to do is have her taken to the vet and put down.

Take her to the vet. This could be one of many problems. I had a cat with similar symptoms and we put her down as it was liver problems and couldn't be cured. We didn't want her to suffer so we put her to sleep.
It could be something thats easily fixed though so its worth going the vets.
