She has ADD really bad. She is on medication and it still shows uncontrollably. I have to endure a whole week with her. She clings on me, stares at me the whole time, follows me around everywere. I have to lock the bathroom doors! How do I make her understand that I am not just one of her teenage friends. I am 22. Sometimes I feel like yelling at her but I can never bring myself to do these things. How do I get things across without scaring my inlaws?

easy! just talk to your in-laws about how your feeling, and remember to do it with love and kindness.
you may also have to talk to your sister-in-law to get her to understand that she's smothering you and you need some space. just remember to do it as kindly as possible. I know she's driving you crazy, but it also sounds like she loves you an awful lot too. maybe you could set some boundaries about where she can go with you and where she can't, and some time limits too. I would talk to my in-laws first, just to make it clear to them, and get them to help you reinforce your rules, and then talk to your sister-in-law about it. I'm sure that you love your sister-in-law, but I also understand the need for a little alone time too!
I wish you the very best.

You did not mention how old the sil is. Why do you have to have her with you for a whole week? You need to talk to hubby about this and then the two of you need to go and talk to your in-laws and explain that it is making you very uncomfortable and that perhaps her medications need to be adjusted as they don't seem to be handling the whole problem. If that doesn't work, then try and stay away from her as much as you can so that it doesn't stress you out.
Good Luck!!
