Birth certificate and social security card, how ever if you already have your learners permit with your picture on it that should be enough. Oh yeah you need proof of insurance of the car you are taking your test in.

depends entirely on what state you live in and what class license you wish to test for. simply type in your state name and drivers license requirements in your search engine and kapow!! That should tell you all you need to know!! Should you happen to live in FL (not sure about the sites for other states) there's even a practice test! Good luck!
