I was certified through the Institute For Phlebotomy Sciences in Modesto, Ca. in 1994=Phlebotomy, ABG's, Cardiac Tech and Lab Tech. I was also the one who also became a proctor teacher with them because of my ability to painlessly stick the young, very young, hard to get, combative, and I have worked successfullu for Met West, Unilab, Kaiser, and now that I want to get back into it, I need me cert. I never got it due to the school disappeared and never sent me my card. I have tried everywhere. I paid $100. for that card and never saw it. All I want to do is work. CAN ANY ONE HELP ME?????? for you to be certified there must be a record in the state in which you were certified
try contacting the state licensing department where you were certified or at your former place of employment as i suspect in order to employ you they would want a copy of your certification

also note if you were certified in 1994
it may have expired it has been 12 years
some certifications have to be renewed and fees paid at certain time intervals
you also may have to show proof of continuing education credits to maintain your status

you will have to bite the bullet and recertify. It's not fair, but that is the fact.
