How to write an accounting memo..?

I'm just wondering what the top of it looks like. I've seen it many different ways.. some with the date on the upper right hand corner, some with the date before the "to", some with the date after "from"... I'm just confused, and I don't know how picky my professor is.......

Please help!

here's the way I've always done them

Top line, centered, "M E M O R A N D U M" (space between each letter)

leave a few lines

then against the left margin:

TO: (tab out) Xxxxx...
(leave an empty line)
FROM: (tab out even with other lines) Xxxxx...
(leave an empty line)
DATE: (tab out even with other lines) Xxxxx...
(leave an empty line)
RE: (tab out even with other lines) Xxxxx...

Leave a few lines and type body...
