It can mean two things (and probably both):
1. The color of the blouse is only white. It has no other colors on it.
2. The cut/pattern of the blouse is simple.

it has nothing to do with the color. plain in this case means unornamented, simple.

This is easy it means no color of any kind on the blouse. Im sure it can have ruffles or white designs on it just no color.

The "texture" or appearance of the garment. It has no lacy collars, no detailed embroidery, no puffed up shoulder pads.

A garment without complication.
