Why do people look puzzled when I say I'm Orthodox?

It's not well-known. Most people are used to Catholic and Protestant and that's it, unless you happen to be in a part of the world where Orthodox religions are dominant.

My significant other was raised Russian Orthodox, and when I lived in Pittsburgh, I knew many people who were Greek Orthodox--it's very prevalent there. If not for those two things, I doubt I'd know much about it either.

It is hard to say just from your question on here. In many religions Orthodox people believe you must dress a certain way, eat certain ways, act certain ways, some can not have physical contact with the other sex (even shaking hands).

So depending on how you live your daily life, and what you are Orthodox in, that makes a difference.

Or it could just be that people don't know what you mean when you say you are Orthodox. Or that it could have so many meanings (Greek, Russian, Orthodox Jew, Etc.). People today are suprisingly uneducated when it comes to religion. In todays climate we should all be more educated about other peoples religions and beliefs.

I think that it is all about exposure. Most people don't know what it means when someone says they are Orthodox!

because unless you say "orthodox what" you are only citing what most hear as a degree, not a faith.

or maybe they are just looking for the horns.
