Ok, here's the truth. The Jeep Grand Cherokee Limited(or any other trim package) has always been known for having a very rough transmission. It has always been a vehicle that shifts very hard. Don't take this as a "nothing's wrong" statement. You could have a problem, but that is not a guaranty. On that body style of Jeep, I believe that manual says to flush the trans every 24,000 miles.(Don't quote me, check your manual) This is much more frequent than most vehicles which is about 60,000 miles. Check how many miles you have on the vehicle and find out how many times this service was done. It would also be a good idea to find out if it had been used to pull a trailer, because this would mean that the service should be done about every 12,000 miles. If it has been serviced properly, you probably are just hearing and feeling the rough, unrefined trans that your vehicle is known for. If that's the case, it's not a big problem. My 99 had the same issue for about 100,000 miles without ever having a "real" trans problem, other than the noise. My suggestion is to drive with a smooth consistant pedal pressure and the trans will shift much more smoothly. One thing to avoid is letting off the pedal just before a shift point, this will cause a very rough shift and a loud clunking noise. Take my word from experience. I had a 99 Limited(same body style as yours) and drove it to 120,000 miles. I'm on my second Grand Cherokee now. Hope this helped you!

Don't listen to those pavement junkies. Take your jeep to a mechanic that you can trust and have your transmission checked. Have them check the fluid and then ask them to check your trans-axle. It's located on your rear tire arm in the back. That fluid needs to be changed periodically also. I think I'm wrong on what it is called but it looks like a big circle between the rear wheels in the back. Get the manual for your vehicle and read the maintenance. Keep it up to date, it sounds like the previous owner didn't and the repairs can be very costly.
Good Luck...

All the Wranglers I have had all had a sort of clunking sound when shifting and never had any trouble with them.
