Saint Agatha, is considered the patron saint of diseases of the breast in legend.
She's the closest you could get to "chronic diseases".
Another contender could be Saint Fiacre (or Fiachra). Born in Ireland in the seventh century, he was better known in France, where he built a hospice for travellers in what is now Saint-Fiacre. His relics are installed in Meaux Cathedral. In Ireland his feast day is 1 September; elsewhere it is 18 August.
Fiacre is listed as the Patron Saint of barrenness; box makers; cab drivers; piles; taxi drivers; sterility; syphilis; tile makers; venereal disease ...
And, of course you have Saint Lazarus. Patron Saint of lepers and leprosy.

St. Lazarus is the patron Saint of those suffering from chronic skin diseases and even today patients go to this Church in numbers invoking the Saint for healing.

As far as I am aware there is no saint that covers all illnesses or diseases. Here are some more specialized saints
* Agatha - breast cancer
* Apollonia - toothache
* Augustine of Hippo - sore eyes
* Blaise - ailments of the throat
* Fourteen Holy Helpers
* James the Greater - rheumatism
* Lucy of Syracuse - haemorrhage, eye complaints
* Marculf - scrofula, diseases of the skin
* Dymphna - sleepwalking, epilepsy, insanity, mental_illness
* Fiacre - Venereal disease sufferers, hemorrhoids
* Teresa of Avila - headaches
* Vitus - chorea
* Simon of Trent - torture and kidnap victims (no longer venerated)

St. Sebastian is the patron saint of pain and suffering, St. Anne is the patron saint of the sick.
