What the patient complains of is that he has a burning sensation town there - this is a result of having unprotected/intercourse with strangers, etc - he has involved his private parts in some form of unwise intimate communication which has brought about a venereal disease... And therefore the saying 'someone must be talking about it' - as in 'if your ears are burning, someone is talking about you' - get the twist now?
Not my type of joke but it's not so hard to understand it ;)

I think this comes from an old saying if your ears are burning they say someone is talking about you. He just tried to be cute and put a different twist on it. He probably thinks you've heard that saying. Its an old one. Don't worry about him....your penis on the other hand......................
