If nothing could change on it, reliability tests could probably be 99.99% accurate but for constantly changing and updated systems it is unrealistic to expect things to always magically go right.

A testing myth: A program is never 100% error free.
Exhaustive testing guarantees only system is error free in those tested environments. if something happens to outside the system, your system only can give detailed report of the bug, can do nothing else.

actually no...after testing your system exhaustively you can only say that your system is the one of the best available in market but can't say it problem free, cause when system runs in actual production environment it faces many new problems/combinations there is always a possibility that at some place it start miss behaving.
