Heat causes matter to expand, causing pressure to build in confined spaces. So, if your blood pressure is already high and you increase the temperature of your body, it heats your blood, which is made mostly of water. Water expands when heated and the limited space within your cardiovascular system is a confined space. So the pressure builds.

because the heat from the hot tub can cause veins to dilate, that along with the high blood pressure can lead to leakage and even strokes or dvt, or oddly enough they can also suffer a hypo- tensive response that leads to loss of consciousness while in the tub and then drowning. (this usually if they are on meds, but not always)

Heat raises your blood flow hence raises the pressure. Not good to sit in hot water with this affliction, could make it worse.
