It sounds like your friend might be schizophrenic. I know people with this disorder and a common symptom are hearing voices and also being convinced that someone is after them. The only thing that helps schizophrenia is therapy and medication.

Sounds like your friend is having some schitzophrenic episodes caused by pain. get him to a doctor, and find the real reason for the pain, and some meds to help. if you are in constant pain, you aren't getting enough sleep, and this can cause what he is going through
