do some lat pulldowns and upright rows. those always kill my upper back. also try some shoulder shrugs.

You need to work out if it is the layer of fat that is making it chunky. If it is then you need to adjust your diet to take in less calories but ideally improve the nutrition, and do some cardio exercise lasting at least 30 minutes 3 times a week.
As you say you already exercise like mad I will assume that the muscle tone itself is quite good, but to stop your muscles getting too bulky lift lighter weights for more repitions ( more than 15).

chin ups
a warning before you start though if you're looking to tone up a certain body part the best way to go about it is lookin at your diet as exercise wont lead to a spot reduction in fat for a given body part

You need to do cardio to get rid of the fat. As for muscle building try doing flys, but lay your stomach instead of on your back.

pull ups.......anything with a pull down motion..........anything with a rowing motion......

but the best way to lose fat is cardio.......although everybody has a place on there body where they store fat naturally.....
