You will have to notify this to your regional passport office by sending a simple application written along with your passport number from where you got your passport. They may contact you to give an affidavid for your claim. Furthur, you will be required to surrender your earlier passport at the office, and submit the reprocessing fee. Thereafter, you will be issued your new passport. Alternatively, you can get your work done by some credible agent, with valid registration number.This way, by, just spending a little bit more, you could get your new passport a bit quickly.

u will need to give the passport office a copy of your legal separation & they will inform u what the process is, whether u need to get a new passport or they can just make a note in your old one.
