Why don't you get her a pedicure/manicure at a spa (even a massage if you can afford it) and ask your fiance or your family to get you one for the same spa. Then you could both go together and make a day of it with lunch and all.
I'm sure she'd appreciate being pampered and she wouldn't have to go alone!

Since your future mother-in-law is very devoted to her family, maybe she would like a picture holder; for instance, at Pier 1, they sell a metal cross that can hold up to 19 pictures or you can try a place like Michael's for a picture holder so that she can have pictures of all her loved ones.

A gift certificate to a spa is a good idea if you go with her! If she's not comfortable with massage, you might get her a facial or manicure/pedicure. Good idea. You are lucky if you get a good mother-in-law!

How about a nicely framed picture of her son? Or maybe one of those collage picture frames where you can put multiple pics of her son, you and her son, and maybe other family pics you might have. Or handwrite a one-page thank you note, being specific about how much you appreciate all she has done for you... then frame the note for her.
