Cut out as much fat as you can, I found that helped me. I also take gaviscon after meals which helps calm the reflux down. Dont lay down after eating. I like to have at least 3 hours before I sleep after my evening meal. If you get reflux in bed sleep propped up on several pillows. I take lansoprazol (15 - 30 mgs depending on symptoms) which is fairly effective.

I've had acid reflux for years :) The only way I can get to sleep is if when I lay down I don't move at all. As soon as I move the acid starts building and my chest and throat feel like they are going to explode. The only other thing that works for me some times is to drink milk. I don't like it but once in awhile it will quell the acid enough to function.

Have you tried modifying your diet? Do a search for "GERD diet" to learn about foods that tend to trigger acid reflux (such as http://heartburn.about.com/cs/dietfood/a/heartburnfoods1.htm).
I'd also recommend you post a message on WebMD's IBS support board - there are a lot of really knowledgeable people there who could help you out, and many of them have a lot of experience with acid reflux. Gve as much info as possible about your symptoms, what you've tried, what tests they've run, etc. The board is at: http://boards.webmd.com/webx?14@@.5987f421

I had GERD for many years.I would buy giant bottles of Tums. Then I went on a diet that included stopping caffeine (coffee and cola) and alcohol (mainly wine). Within 2 weeks my acid reflux symptoms were gone. I'm sure this does not work for everyone, but if you have not tried it, it's worth a try.

I use Nexium myself when I eat something spicy or with red sauce. Maybe try Prevacid or Prontonix... Have you been to a Gastroenterologist? You might need an EGD (Esophagogastroduodenoscopy) done...then they can tell you EXACTLY whats wrong. Good luck! : )
