Depends on what you are applying the term to.
For cooking as in 'rare steak' it means one that is cooked only a little, outside seared, inside still red.
For most other uses, the word rare denotes frequency = not very often, infrequent.

It depends on the context within which it is found. I would suggest rare affects 1% of the population or less, out to about 1000th of people. A disease affecting only one out of a thousand or fewer would be exotic; but really you have to ask whoever used the term.
And remember, 1 in 1000 would be 300,000 in the U.S.A. .
