There's not enough information to give you any advice. We don't know why you two broke up. Did she break up with you or did you break up with her? Why did you break up? Did you cheat? Did she cheat? Did someone lose interest?
The best advice I can give you based on what I know of the situation (which is absolutely nothing) tell your ex girlfriend how you feel. Tell her you miss her and want her back and apologize for anything you did (if you did in fact do something.).

you cant unless she wants too.
just be nice to her, say she looks nice etc, if she asks what part say her hair eyes lips etc. its a start
