For practicality, you should go with a duvet cover and a down comforter. The comforter will keep you warmer, and the duvet cover can be easily removed and washed (it's like a giant pillowcase). Down comforters can cost quite a bit, especially if you're looking at the higher end ones. I recommend one that does NOT allow the down to be shifted - it works its way to the bottom/edges and it's a pain to try to shake and beat back into place. You can get cheaper comforters, but this is something that could last for many years so it's worth spending a little more Also, if you get tired of the look, you can just buy (or make) a different duvet cover for a lot less than replacing the entire thing.
A quilt is your better bet for looks, and a high quality one can be as warm as a down comforter, although it will probably be heavier. Quilts can be washed, but it is a bigger chore than a duvet cover. A hand made quilt, heirloom quality, can last decades if cared for properly. Two of my grandmother's cousins are still using quilts they made for their weddings 60 years ago. A well made quilt big enough to cover a bed can cost as much or more than a down comforter and a duvet, though.

A duvet cover is just that. It's purpose is to cover an existing quilt or comforter. So, with that being said, if you already have a good warm comforter or quilt and want to change the look of it, go for the duvet. The plus side is that you can remove the duvet cover for easier laundering. Good Luck!

i've got lots of quilts and i've never washed them, just recovered them. I'd go with the duvet.
