Be sure you inhale into the bottom of your lungs as deeply as possible. It's when your oxygen level starts down that you begin to tighten up.

it's all diaphragm and support!!!
your throat shouldnt be tense anyway. that probably means youre not opening it up. your closing your throat off not letting the air or the sound come through.
play with your voice alot. i just got to a point where i can belt some very high notes and i honestly dont even know what i did. i jusat worked out my diapraghm a LOT and made it strong and really focused on using it.
And breath too. make sure your fills up your lungs from bottom to top when you breath, you'll get more air that way.

first of all, make sure you've relaxed your face muscles by stretching and tapping/massaging your face and neck. next, focus on doing some deep breathing by placing your hands on your stomach so you can be aware of what you're naturally doing.
then, do some vocal warm-ups that make you aware of your range and projection, like saying YO! really loud, going from your highest note to your lowest note. then do the reverse with the word NO!
other than that, get yourself a good vocal warm-up tape/CD that'll push your range...

this might sound weird but it will work, first take a deep breath then make a small hole with your lips and release it slowly, I said slowly. you have to do it at least say 10 times a day for a while before you can tell a difference. next pick a song that you want to be able to hit, and first sing it as low as you can, and yes you will laugh at yourself few times. next sing it a bit higher then in your own talking voice then singing voice and so on. you don't have to do it all in one day. this will really increase your range.so good luck and have fun with it.

for breathing: breath in through your nose like you're smelling cookies and inhale all the way down to your crotch. to support the high notes use your "puke" muscles.
for the high notes: open your mouth wide and make sure you have enough air to support it.
