ROUGHLY speaking, cavity pain progresses as follows:
1. Cold sensitivity - small to medium hole. Probably able to be repaired with a filling.
2. Hot sensitivity - medium to large hole. May be very close to the nerve and require a Root Canal Therapy or extraction.
3. Hot sensitivity with throbbing - RCT or extraction.
4. Dull ache - tooth nerve (pulp) may be dead and infected, and you are feeling the development of an abscess. RCT or extraction.
The longer you have had the pain, or the more intense it is, or the longer it lasts each time it affects you, then the more likely an RCT or extraction.

"Constant dull pain, but not bad" is not good. That is often the first hint that you need a root canal.

It could be possibly be a gum infection and not a problem with a tooth.
