My nephew is only 3 & 1/2 and I've noticed whenever he holds something his hands are very shaky. Could this be anxiety or nervousness, or perhaps even something more serious?

It could be psycological but it is far more likely it is neurological. I would get him into a pediatrican and let the doctor check him out.

It could be a medical problem that needs to be tested for. Could be a birth defect or even diet. I know I have a sugar problem and if I don't eat at the right time I get shaky

Definately get him to a doctor! In the mean time, take him off all sugars, sodas, and anything with artificial sweeteners and anything containing high fructose corn syrup..including nutra-sweet. All artificial sweeteners are synthetic, toxic chemicals, and he could be having a reaction to the sugar substances. Please take my advice, and get this child on an "organic" diet. and take him to the doctor!

My son did that until he was 2. Especially when he just woke up. I blamed myself cuz I smoked while I was pregnant with him & I drank a lot of caffine. He did outgrow it. If it is severe, I'd definitely call the doctor. Best wishes.

I would suggest you see a dr about this. It could be serious, and it's better safe than sorry.....
