Once the foreigner is married to an American, the American can - but doesn't have to - apply for permanent residency for him by filing an I-130 with USCIS. Once that is approved, the foreigner can either apply for an immigrant visa (if he is outside the US) or for adjustment of status (I-485, if he is legally in the US).

If the immigrant visa is issued, the foreigner will recieve the green card in the mail some time after having entered the US as a legal immigrant. If he applied for adjustment of status, the card will also be mailed to him eventually.

Naturalization and citizenship can come along later. Marriage does NOT grant citizenship. That must be applied for by all of its own unique rules.
