Sounds like you need a bite guard. You can get a custom one from your dentist or a generic one from your local pharmacy that will work just fine. You might also have TMJ, in which case you will need surgery.

I'm curious why he didn't make a bite plate for you to wear at night if he thought you were clenching. What are you going to do, take a muscle relaxer for the rest of your life because of a clinched jaw? Go to another dentist.

Can't tell much from your question, but if he gave you antibiotic, must be infection somewhere there.... If you grind your teeth, you can make your teeth and jaws hurt, as well as damage the enamel on your teeth. If you're hurting that much, call dentist and ask for pain med prescription. If you can take tylenol, it might help. Also warm wet cloth on your jaw might help relax the muscles. It may, however, require a stronger pain med than tylenol, which you'd have to get on prescription.
